Exemplar Care Management is seeking a Community Support Worker to provide individualized, person-centred support to adults and youth within community-based settings. This role focuses on promoting independence, enhancing quality of life, and supporting individuals to participate meaningfully in their communities.
Support is delivered in alignment with individualized care plans and may take place in community spaces, supported living environments, or during daily activities.
Provide one-to-one support in community and home-based environments
Support individuals with life skills, routines, and daily activities
Promote independence, dignity, and informed choice
Assist individuals in accessing community resources and activities
Maintain accurate documentation and progress notes as required
Communicate effectively with supervisors, families, and team members
Follow individualized support plans, safety guidelines, and organizational policies
Relevant education or experience in social services, health care, or support work
Experience supporting individuals with diverse needs is an asset
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Valid Class 5 driver’s licence and reliable transportation
Clear Criminal Record Check
First Aid & CPR certification (or willingness to obtain)
Ability to work flexible hours as required
