Position Summary

Exemplar Care Management is seeking a Home Support Worker to provide in-home, one-to-one support to individuals who require assistance with daily living activities. This role focuses on promoting independence, safety, comfort, and overall quality of life within a home-based environment.

Support is delivered in alignment with individualized care plans and may involve personal care, household support, companionship, and assistance with routines essential to daily living.

Key Responsibilities

  • Provide one-to-one in-home support tailored to individual needs

  • Assist with daily living activities such as personal care, meal preparation, and light housekeeping

  • Promote independence, dignity, and personal choice

  • Support clients with routines, mobility, and community engagement where appropriate

  • Build respectful, trusting relationships with individuals and their families

  • Follow individualized care plans and document support activities as required

  • Maintain a safe, clean, and supportive home environment

  • Communicate effectively with supervisors and care team members

Qualifications & Requirements

  • Relevant education, training, or experience in health care, social services, or support work

  • Experience supporting individuals with diverse needs is an asset

  • Strong communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Valid Class 5 driver’s licence and reliable transportation

  • Clear Criminal Record Check

  • First Aid & CPR certification (or willingness to obtain)

  • Physical ability to assist with daily activities as required

  • Flexibility to work varied shifts, including evenings or weekends if needed
