Position Summary

Exemplar Care Management is seeking a Program Coordinator to support the effective delivery and coordination of community-based support programs. This role plays a key part in ensuring services are delivered in alignment with organizational standards, funder requirements, and individualized care plans.

The Program Coordinator provides operational oversight, staff support, and coordination between clients, families, support workers, and external partners to ensure high-quality outcomes for individuals receiving services.

Key Responsibilities

  • Coordinate and oversee the day-to-day delivery of assigned programs

  • Support, mentor, and guide frontline support staff

  • Assist with staff scheduling, coverage, and resource allocation

  • Ensure services align with individualized care plans and organizational policies

  • Act as a point of contact for clients, families, and community partners

  • Monitor service quality and support continuous improvement initiatives

  • Maintain accurate documentation, reporting, and program records

  • Support onboarding, training, and performance development of staff

  • Participate in team meetings and contribute to program planning

Qualifications & Requirements

  • Diploma or degree in Social Work, Child & Youth Care, Human Services, or a related field

  • Relevant experience in community services, health care, or social services

  • Previous leadership, coordination, or supervisory experience is an asset

  • Strong organizational, communication, and problem-solving skills

  • Ability to manage multiple priorities in a fast-paced environment

  • Proficiency with documentation, reporting, and basic computer systems

  • Valid Class 5 driver’s licence and reliable transportation

  • Clear Criminal Record Check

  • First Aid & CPR certification (or willingness to obtain)
