Exemplar Care Management is seeking a Home Support Worker to provide in-home, one-to-one support to individuals who require assistance with daily living activities. This role focuses on promoting independence, safety, comfort, and overall quality of life within a home-based environment.
Support is delivered in alignment with individualized care plans and may involve personal care, household support, companionship, and assistance with routines essential to daily living.
Provide one-to-one in-home support tailored to individual needs
Assist with daily living activities such as personal care, meal preparation, and light housekeeping
Promote independence, dignity, and personal choice
Support clients with routines, mobility, and community engagement where appropriate
Build respectful, trusting relationships with individuals and their families
Follow individualized care plans and document support activities as required
Maintain a safe, clean, and supportive home environment
Communicate effectively with supervisors and care team members
Relevant education, training, or experience in health care, social services, or support work
Experience supporting individuals with diverse needs is an asset
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Valid Class 5 driver’s licence and reliable transportation
Clear Criminal Record Check
First Aid & CPR certification (or willingness to obtain)
Physical ability to assist with daily activities as required
